Any student that needs to take over-the-counter medication and prescription medication while at school, needs to have a medication administration paper filled out and signed by a doctor and parent before medication will be administered at school.

Dear Parent/Guardian

If your child is sent home from school with a fever or has a fever in the evening or night, please keep him/her at home.

Your child needs to be fever free for 24 hours without the use of fever reducing medications such as Tylenol or Motrin before returning to school.

Thanks for keeping our school healthy!!

New Ohio Department of Health Immunization Requirements

The Ohio Department of Health (ODH) has made changes for Ohio school children in regards to immunization requirements starting with the 2016 - 2017 school year.

All 7th graders will be required by the State of Ohio to have proof of the Meningococcal (MCV4) vaccine and TDaP on file BEFORE the start of 7th grade.

All 12th graders will be required by the State of Ohio to have proof of the Meningococcal (MCV4) vaccine on file BEFORE the start of 12th grade.

The meningococcal vaccine must be given after the age of 16 years old. If your child had a dose of MCV4 before the age of 16, they are required to have a 2nd dose.

Students will not be permitted to attend class without an updated shot record showing the vaccine administration date. It is the parent's responsibility to bring, send, or fax the updated copy of your child's shot record to school.

Please schedule an appointment with your primary care physician, visit your local health department/Wal-Mart/Kroger/CVS.

Please do not procrastinate getting your child's required immunization. Doctor appointments will fill up fast so do not delay and wait until August.  Your child will not be permitted to start school and days missed will be counted as unexcused.

As soon as you take your child to get his/her immunization, please bring, send, or fax the information to 740-820-2466, attention School Nurse.

If you have any questions, please call Deborah Daniels, RN, Minford Local School District at 740-820-3009.
