* Fees $50.00 for 1 player, $70.00 for 2 players, and $85.00 for 3 or more players.

* Needed... 3 proofs of residence dated on or before Feb. 1st, 2015 (utility bill, report card, drivers license, and auto registration are some examples).

* Needed... Players Birth Certificate
